The LTCF Vaccine Toolkit has been developed as a resource to help Long-Term Care (LTC) residents and staff with access to COVID-19, flu, and other recommended vaccines. The following are vaccine access pharmacy resources.
- LTCFs are encouraged to reach out to their regular or contract pharmacy to ask about COVID-19, flu, respiratory syncytial virus (RSV), pneumococcal and other vaccination services, including on-site clinics.
- LTC residents and staff who can travel to a vaccine site are encouraged to utilize vaccination sites in their local community (i.e., nearby pharmacies or healthcare provider offices); this may be the fastest way to get vaccinated.
- If an on-site vaccination clinic is the best option to meet LTC residents’ needs, facilities can contact LTC or retail pharmacies directly for support. When planning, LTCFs should consider how many residents and staff will need vaccines. Also consider planning co-administration of flu, COVID-19, and other recommended vaccines.
Long-Term Care Pharmacies
- If you do not have a contracted pharmacy provider, there are participating LTC pharmacies that may be available to service your facility’s vaccine needs. Facilities can reach out directly to a nearby LTC pharmacy of their choice to ask about vaccines. Note that while some pharmacies are only serving contracted facilities, others are willing to service outside their usual contracted facilities. The timing and availability for vaccine services will vary based on the pharmacy resources and facility location.
- The two largest LTC pharmacies are Omnicare and Pharmerica.
Omnicare
- Distribution model — subcontract arrangement with the facilities to distribute vaccine and utilize facility staff to do the vaccine administration. Omnicare would manage vaccine supply delivery logistics and vaccination reporting to the California Immunization Registry (CAIR).
- Ambulatory model — residents and staff of LTCFs can call 1-877-652-0918 (Monday-Friday, 6AM-3PM), a support line dedicated to assist LTCF residents and staff to schedule an in-store COVID-19 vacine appointment at a nearby CVS location.
- On-site clinic model only for assisted living facilities (not skilled nuring facilities) — full service on-site vaccine clinics are available via a CVS pharmacy-led clinic model. Current criteria are a minimum of 45 immunizations in any combination per clinic and facility location must be within 30 miles of a CVS pharmacy.
- If you are currently a customer with Omnicare, we encourage you to continue working with Omnicare for your vaccine needs.
Pharmerica
- Distribution model — subcontract arrangement with the facilities to distribute vaccine and utilize facility staff to do the vaccine administration. Pharmerica would manage vaccine supply delivery logistics and vaccination reporting to CAIR.
- On-site clinic model — a full service of on-site vaccine clinics including vaccine delivery, administration, and reporting. Facilities may directly contact ImmunizeLTC@pharmerica.com.
- If you are currently a customer with Pharmerica, we encourage you to continue working with Pharmerica for your vaccine needs.
Community Resources for Appointments
LTC staff and residents who are mobile should seek vaccine from community resources including pharmacies, healthcare providers, and local health department clinics; this would be the fastest and easiest way to get vaccinated. There are multiple online resources to locate and schedule an appointment. Transportation services to and from appointments may be available from health insurance plans, including Medi-Cal.

Medi-Cal offers transportation to and from appointments for Medi-Cal beneficiaries for services covered by Medi-Cal. Use this poster to help communicate the Medi-Cal transportation benefit.
California Department of Aging contracts with and oversees the Local Area Agencies on Aging (AAA) that coordinates transportation services for 60 years of age or older regardless of income level. Locate AAA services in an area by calling 1-800-510-2020 or by using the Find Services in My County page.
On-site Vaccine Clinics from Retail Pharmacies
Many retail pharmacies also offer on-site vaccine clinics (COVID-19, flu, and other vaccines) if certain requirements are met; interested facilities can reach out to pharmacies directly to inquire about vaccine clinics. Note that timing and availability of on-site clinics will vary based on facility details and pharmacy resources. When planning these clinics, also note that flu, COVID-19, and RSV vaccines can be co-administered. The links and contact information below cover how to request a vaccine clinic.
CVS
- A minimum of 45 immunizations is required per clinic (any combination of vaccines given).
- If minimum number of participants cannot be met, vaccine vouchers are available to be redeemed (see link for details).
- All locations must be within 30 miles of a local CVS Pharmacy.

Walgreens
- Minimum number of immunizations required for an onsite clinic (may include a combination of COVID-19 and flu vaccine) is 50 required per clinic.
- Locations may vary. Submit LTCF Vaccination Interest form and a representative will respond.
- Walgreens Community Clinic Events
- Requirement is that all members of the community are invited to participate. Events must be open to members of the general community in addition to LTCF residents and staff.
- No minimum number of immunizations is required if event is open to the public
- Can also provide flu vaccines to uninsured persons.

Rite Aid
- A minimum of 20 participants is required per clinic per hour. If a facility has fewer than the 20, Rite Aid can still assist if the facility agrees to pay an hourly fee of $100.00.
