For VFC, VFA & LHD 317 Providers
Every Provider of Record & Designee and Vaccine Coordinator & Backup must complete EZIZ training to meet the educational requirement. (All immunization staff, however, are encouraged to complete the training.)
The lessons below must be completed annually for Recertification. Providers may not recertify until the training has been completed.

Information needed
- Your practice’s PIN and zip code
- Your EZIZ username and password (if you have previously taken EZIZ training)
Take the training
- Log in to EZIZ training
- Complete required lessons
- You will need to reset lessons if they were completed before December 1, 2023
After training is completed
- Login to myCAvax and click on the Recertify Location button
